disaster I Guess He Did Something Wrong

Power plant worker here. Looks like the maintenance worker was rolling a breaker back into its panel on the switchgear. Either it was energized at the time of it being rolled back in which is a big no no, or a tool or something metal was left inside of it that caused it to go phase to phase and explode. Worst Arc flash Ive ever seen tbh. Dudes luckier than a 4 footed leprachaun hes alive

I can never understand why someone else is video recording when these things happen. Did he invite someone to record while he replaced that rolling circuit breaker. if so, why?

Ive personally not worked in a plant that utilizes body cameras, some plants do use them on maintenance workers doing critical maintenance for liability reasons and for safety. Basically if someone fucks up you cant point at the dude behind you and say, "He told me to do this" or "It just happened for no reason." It helps them understand why something goes wrong if it does so it doesnt happen again.
