The angry ghost took time to get the direction and level of the puncture just right.

If you look closely, you can see he directs the blade laterally (to the outer side) towards the armpit (axilla) which would mostly avoid the lung. It would potentially break the vacuum of the chest cavity causing a pneumothorax (air around the lung) and likely cut the lung itself.

But this direction should miss any larger blood vessels. I suspect he wanted transfer to the hospital with view to a new prison after being ‘attacked’.
