Dwarf Fortress easily. Sure it's an ASCII game, but the engine for it is really good and the way necromancy works makes it even worse.

One time I tried to disembowel and butcher every body I found outside of a necromancer Tower. Then the necromancer went outside and all of the flesh turned living and drowned my character alive. He was ran down and covered in blood and gore, panicking, I saw warnings for my endurance, he was exhausted from struggling and suffocating in a sea of living skin. My intention was that if he had no full bodies to raise, surely there won't be zombies. I prefer the zombies.

Even in just the base game it's very gory. Bits come flying off, you can get nerve damage that fuck up the function of everything. Going forward. If you lose a finger, maybe you get disarmed by your weapon easier. It's the first game I played where small real injures from battle accumulate In a way that makes yourself adjust how you have to fight.

If the first game I ever played a horse kicked one of my dwarves and his heart exploded on impact, collapsing immediately dead. Very fun game.
 
Gears of War impressed me when it came out 15 years ago. I love the way bodies rupture from close range shotgun.

Rdr2 has realistic body and head wounds just not when you completely blow someone’s head off with a pistol 10/10 game
The moment I fell in love with the game was when I blew a guys lower leg off with the sawnoff shotgun, it did the black and white cut scene thingy and then he slowly fell over.
