There's a hatch on the ceiling of most elevators. If you can reach it you can climb out onto the top and then use the elevator shaft ladder to climb to the nearest landing. To open the doors you need only press a small lever and it opens automatically. Problem is all the variables in different elevator designs means this won't be as straight forward in reality.
