But being serious, answering your question...I reckon some people are born with a chemical imbalance and are psycho from birth...Most people know someone that had all the love and affection and surroundings to thrive, but ended up some dead shit criminal...
On the other hand, Your surroundings and life experiences and upbringing can shape your application of empathy towards other humans....Like my ol' man used to say, you wanna survive in a violent world, you have to be violent yourself...
Look at media and music...Most thoughts we have, are actually not our own original thoughts and have been molded by music, media and the education system...

That's why I'm curious on the subject. I find my feelings on it confusing. 😐 Of course it depends on the relationship I have with the person. for example I recently worked a recovery program helping newly sober addicts with paperwork, applications, ect... I really did care about helping these people better their lives but after repeatedly just taking advantage of the program and resources and just showing no desire to get better, it became more than easy to write that person off completely.

Lol I have 5 cats don't need anymore. I did help alot of addicts who are still sober and they still seem very grateful. I don't have sympathy for the ones who choose to blame their addiction on others and make ridiculous excuses as to why they keep torturing themselves. Most will take no responsibility because "the disease" makes them do it. 🙄 It sounds cliche but of course not all addicts are the same. I do alot of kind things and care about alot, I do go out of my way to make sure the ones in my family are happy and comfortable. I guess it's circumstantial to your surroundings and all.
 
Empathy should always be selective and optional. If someone rights you, show them empathy and make a good friend. If someone wrongs you, deny them empathy and do not associate with them. Everyone does this and the only people who call you a psychopath are the ones you wouldn't want to empathize with in the first place.
 
The reaction you explained is normal most people think we are sick because they live sheltered lives I got into things like this from studying and preparing myself for career paths I was taking I wanted to be an EMT for a while and ended up doing something a little more intense, but most people on here are realists we live and we understand seeing this disensitizes us to the real world the difference between me and a person who thinks I'm a sick freak is, if someone got shot I would immediately move to apply pressure to the wound and find out where the wound is and how lethal it is and the people who r sheltered and think I'm sick freak out crack under pressure or pull out their phone so that begs the question who is really sick. We have different amounts of empathy for certain things empathy is more around what u know and your ability to put yourself in that person's place even some serial killers have empathy sometimes, not for victims, but say they watch a kid living the same horrible childhood they had they know the feeling and have the ability to show empathy towards that; but sympathy they lack they don't feel bad for people. I think what your looking for more is the differences between empathy and sympathy. Just wondering if this helped in any way.
