Belayer not anchored so as to send weight of climber to the anchor(prefrably behind belayer).
Belay device suspect, might be low quality or unsuitable for pitch climbing. Anchors placed at long distances. Inexperienced climbers.
Hopefully they enroll in a climbing course and learn good safety techniques.
