Probably what happened. He had a dirty, gummed-up bolt. He was pulling the trigger and it released the firing pin but was hanging up. The firing pin channel was probably gummed up. The last time he did it, he plopped the gun on its side, on the table and the jarring was enough to finally release the firing pin.
