Have any of the facilities you work removed the wrong appendage? The US Veterans Affairs Health system Is known for that. There was even a case where they amputated a leg that had written on it “Not This Leg!!” shortly after the implementation of the new rules requiring the surgeon to sign the affected appendage.

As the story goes, the surgical resident responsible thought it was a joke and somehow convinced the surgical assistant to prep it. The senior attending had no idea about the conversation…now there is a veteran who survived Vietnam intact has no legs.

Today there are so many write/sign/communicate rules it shouldn’t happen again, but with VAH who knows.
