Imagine if after he said he was fine nobody helped him and he just stayed at the bottom of the escalator all day slowly being chewed up by the steps. And then when the store closed for the day the janitor dragged him off to the dumpster behind the store, and then rats among the trash began biting and shitting on him, and he was still alive but paralyzed so couldn't do anything, and then the trash men came in the morning and tossed him in the back of their truck, then at the landfill dumped him on a pile of medical waste from a hospital, and then these aborted fetuses from the medical waste began crawling all over him and one had a scalpel and cut open his stomach and all the fetuses went inside, then months later the fetuses emerged as trash gremlins - a new race of human mutants, and slowly but surely these creatures conquered Earth. Also the dude in the wheel chair was still alive somehow after all this but was a necrotizing skeleton monster of sorts that the trash gremlins worshiped as a god and sacrificed normal humans they captured to him on a giant fire. That would be cool.
