What the hell is wrong with these people, that was like a 4 feet walkout away from the rack and the whole place was crowded to begin with, a bench right in the way that he dropped the barbell onto, lots of machines around him that he could have fallen into if he fell backwards. Whole gym is a deathtrap and the people there are retarded.
