I was around 10 and my brother had a book about movie gore ( yea i was that gore magazine and fangoria reading creep even when I was a kid ) and the first really gore picture in it was from a movie ( does that count? ) Some broad was hanging naked from a wooden structure impaled by 2 big ass steel hooks one threw each of her boobs.

Slightly off topic, but when I was (too) young I loved reading Reader's Digest magazine, which always had a section called Drama in Real Life. No pics obviously but first hand description of everything from skiing accidents, building collapses to grizzly attacks! In one copy there was also an advert for Amnesty Int or similar, which listed various methods of torture. And so it began...

My first Gore thing I saw was when I first got the Internet and was into all the p2p file sharing
It was a B&W video titled "unknown Russian soldier" and showed a man laying on the floor with someone putting there foot on his head and putting a knife into his throat and cutting outwards, I remember watching it around 50 times just listening to him struggling to breath and gurgling on his own blood

Before all that there was the faces of death box set but I don't class them as real so it's the slit throat video

I have seen many other disgusting things on the internet and guess what..... I'm still normal, I haven't killed or tortured anyone or anything thing and I lead a normal life with 2 great normal children

Possibly like many others, my first "gore" image was probably Christ on the cross. My first gore photo was the black 'n whites of that kid that had been raped and killed, later to be found half decomposed. That was back on Rotten.
