From what I have heard (from people who do like their job in this field) it’s mostly about careful restraint + doing it with multiple people. And sometimes a patient just isn’t in the mood and will be left alone for a bit and then try again.

This has been a huge problem for a long time. It's always the niggers, they have no ability to work in such positions but it's the fault of shady private Owners of these places. A lot of the facilities don't meet proper standards and they know that niggers don't care about standards so that's why they hire stupid clueless black people so they can keep taking short cuts - Sad Truth.
Also, these niggers get the work through agencies, they never pass any vetting checks - while the agency takes a cut from Government incentives etc ..the whole thing is dodgy as hell.
