Dogtooth - Disturbing, about a controlling mother and father, with oblivious daughters, that's just the basics.

And... The Girl Next Door. I actually felt anger and disgust watching this, what she was put through is just terrible. The victim showed no signs of doing anything bad to anyone, genuinely a good person. In my opinion, if you're looking for disturbing, this is what you want ..

It was quite difficult for me to list obscene films with no gore. My first one is a classic. The rest contain some gore. Nevertheless, these are the most obscene films I've seen in no particular order after the first one.
Tod Browning's Freaks (1932) Pre-Code Hollywood (1927-1934) No gore.
Bob Guccione's Caligula (1979) It has some gore, considering it takes place during Emperor Caligula's reign in 1st century Ancient Rome, but tame by today's standards. The original screenwriter was Gore Vidal.
Pier Paolo Pasolini's Salò, or the 120 Days of Sodom (1975-76 European releases) It has some gore, and is grotesquely obscene with torture, depraved sex, coprophagy and executions.
Takashi Miike's Audition (1999 Canada release - 2000 Japan release) It also has some gore.
Takashi Miike's Imprint (2006) Some torture gore, not much. Psychological Thriller
Tom Six's The Human Centipede (2009) Some surgical gore.
Franklin J. Schaffner's Papillon (1973) Depicts the brutality of the early 20th century French Guiana penal system and a prisoner's eventual escape. Not much gore.
T.F. Mou's Men Behind the Sun - Chinese Black Sun: 731 (1988) A graphic depiction of the war atrocities committed by the Imperial Japanese at Unit 731, quite gory, highly obscene showing human experimentation including vivisections, administering biological weapons, cold, pressure, and explosion testing.
