In many jurisdictions the regulatory body responsible for the inspection of fair rides (which are usually itinerant in nature so there aren't full time inspectors) are the same people responsible for checking elevators and escalators. I'm sure they do their best but still.

And then you have the mutant carnies who assemble and operate the rides. I'm fairly certain that they're the same the world over, weirdos who have a hard time tying their shoes but are expected to maintain and operate fast moving equipment.
