Simply put, there's a release valve that the nurse must turn for the pressurized air to escape the cuff. If that release valve is fully removed, the cuff can't hold air.
So, why not create a similar device that can be placed loosely around the neck that would accomplish the same goal that the "choking game" enthusiasts are after?
There wouldn't need to be a release valve at all. Instead, one would just keep their thumb pressed over a hole in the air bladder, thus keeping the air in. The slightest relaxation of the thumb would cause the air to rapidly escape and that's the entire idea in a nutshell. As soon as the user passes out, their entire body begins to relax. In fact, it's a deliberate response initiated by the brain.
So, you'd get to experience an orgasm that is supposedly better than the best one you've ever had, all at the small expense of a few brain cells but without the potential embarrasment of your accidental masturbation suicide ever making it onto the internet.
