My Uncle died like that. Many years ago, when I was a kid, there had been an explosion and a gas leak at his factory, in the mass exodus to leave he'd banged his head on a metal beam, and some other bloke caught him before he fell over the metal railings, but, in the rush to save himself, left my uncle propped against said railings, he slipped through and fell head first to the ground, he never regained consciousness and died in a coma a week later
