In the US, the hospitals owned by charity organizations, such as churches, will provide treatment for indigent people like this and the hospital will send an application to the state Medicaid (welfare) system for him for free insurance. The problem with this guy is he doesn't want to detox in a hospital and wants to go to a step down or rehab program first before he goes to the hospital, which is not possible due to his state of health. His addiction is stronger than his will to live at this point. Hopefully he will hit his "rock bottom" soon and accept help whatever help wherever he can, but he's probably septic at this point and his death is certain if he doesn't do it ASAP Rocky.
