You are referring to respiratory secretions or “death rattle”. When a dying person’s conscious level drops into a coma in the final stages of death they can’t control the saliva and mucus that is being secreted by the glands. The liquid pools in the back of the throat and upper airway creating a gurgling/ rattling/ bubbling sound as they breath. Even in sleep we can swallow or cough liquids away from the airway, but not if we are in a coma or in the last stage of death.

I prescribe injectable Buscopan (Hyoscine Butylbromide) to dry up and reduce the death rattle but this is done to make it easier for the relatives as the patient is too unconscious to experience any discomfort from it. It’s hard to manage in patients with heart failure as they often have pulmonary oedema (fluid in the lungs).

I'm thinking he was trying to stand back up after laying down. Unfortunately, the way his legs are shaped he would have no other option but to lurch backwards to get himself upright if he couldn't lower his head for counter balance and obviously he doesn't have arms. Sucks to have backwards knees. I feel bad for the guy.
