bizarre A 19 years old homeless drug addict shows the state of his body.

Incorrect. An ER is obligated to treat someone with an emergent medical condition (as this clearly is) regardless of ability to pay. This fact is very well-known in the US.
I think you’d be very shocked on how many hospital refuse homeless and have them walked off the property by security here in the US lol especially in states with poor medical to start

They must legally treat *emergent* medical conditions. They do not have to treat non-emergent conditions, nor humor homeless drug seekers. Hospital administration is very aware of liability and what financial damage they can incur if they refuse to treat someone who then immediately dies from the condition for which they sought treatment. That is rare. If you see someone being escorted out by security, it's extremely likely they were disruptive and/or drug-seeking, AND didn't have a real medical emergency either. They will let them sit there usually if they're not disruptive.

Going to the hospital means he wont be able to get a fix. It seems ridiculous to non addicts...but his addiction is so strong he cant bear the thought of doing anything that would put a barrier up to getting drugs...even medical care that he desperately needs. He doesnt care if he dies. Or if his leg falls off. He only cares about drugs.
this is sooo true...
i remember when i used to smoke cigarettes i would avoid places like the hospital like the plague because you can't smoke there, especially if i thought they were going to keep me in for a while
...and everyone else i know is the same way, with some of them it's about smoking weed or popping pills they have no prescription for...

I don't understand how they let it get that bad! Fair enough you have a drug habit and probably mental health issues but that dont stop you from going to hospital.
It’s way more common in the US than people think, unfortunately. If you don’t have the money, they will let you die in the street. Not all hospitals, but a lot. Which is exactly why healthcare should never be privatized. Healthcare should be a human right.
